AGENDA PLEDGE OF ALLEGIANCE PUBLIC HEARINGS 1. 1107 Old Country Road – Public Hearing for a Site Plan Application entitled 1107 Old Country Road which seeks approval to construct to a 2,295sq. ft. medical office building with on-site parking and other related site improvements within a 1.35-acre parcel which is currently improved with an approximately 9,300 sq. ft building, located within the Shopping Center (SC) Zoning Use District, situate at 1125 Old Country Road, Riverhead more particularly identified as SCTM No. 600-108-4-7. Lizem Associates, LLC – Public hearing for the minor subdivision application of Lizem Associates, LLC, seeking to subdivide an existing, vacant, 111.38-acre parcel of land located on the northeast corner of the intersection of Middle Country Road and Fresh Pond Avenue in Calverton, with the subject parcel being located in both the Rural Corridor (RLC) and Agricultural Protection (APZ) zoning use district, and more particularly described as SCTM No. 600-98-1-1.1. RH Hamptons LLC. – Discussion of Long Island Workforce Housing Act sketch for the subdivision application entitled RH Hamptons LLC which seeks approval to subdivide a 28,741 sq. ft. vacant parcel into five (5) lots with frontage on Union Avenue and Maple Avenue, to the north of Long Island Rail Road., and is located within the Downtown Center 5 Residential (DC-5) Zoning Use District, situate at 308-310 Maple Avenue, Riverhead, more particularly identified at SCTM No. 600-129-2-46.1 (Matt Charters) 4. Bargain Moulding – Discussion of a site plan application seeking to perform access and parking improvements at an existing vacant commercial site located at 1133 Old Country Road, Riverhead, in order to reuse the existing building as a molding, window, and door retail showroom and warehouse. The subject properties, identified as SCTM Nos. 600-108-4-6.4 & 6.6, are located on the south side of Old Country Road, within the Business Center (BC) zoning use district. Resolution No. 2025-017 – RH Hamptons LLC – Resolution approving Long Island Work Force Housing Sketch and designates Lot 3 as Workforce Housing Lot for Appraisal Purposes only for the subdivision application entitled RH Hamptons LLC which seeks approval to subdivide a 28,741 sq. ft. vacant parcel into five (5) lots with frontage on Union Avenue and Maple Avenue, to the north of Long Island Rail Road., and is located within the Downtown Center 5 Residential (DC-5) Zoning Use District, situate at 308-310 Maple Avenue, Riverhead, more particularly identified at SCTM No. 600-129-2-46.1 (Matt Charters) 2. Resolution No. 2025-018 – Atrium Café – Resolution granting preliminary approval, with conditions, for a site plan application which seeks approval to construct a 2,382 sq. ft. restaurant with 34 indoor seats and 8 outdoor seats, along with associated site improvements including a sanitary system, site drainage, parking, paving, lighting and landscaping on a 17,311 sq. ft. Resolution No. 2025-019 – Cox Place of Worship – Resolution classifying an action as Unlisted pursuant to SEQRA and issuing a Negative Declaration for a site plan application seeking approval to convert an existing single-family dwelling into a place of worship and add a 3,600-plot church yard, along with site improvements including a one-way access road, parking and paving, a new sanitary system, on site drainage, lighting, landscaping, and an ADA-complaint path and ramp on a 216,991 sq. ft. (4.98-acre) improved parcel, located at 5284 Route 25A in Wading River, within the Residence B-80 (RB-80) zoning use district, more particularly identified as SCTM No. 600-76-2-4.
